In the second session of the Population Health Learning Series, we learn how to collect and apply information to improve the health and well-being of our community. This session focuses on the following: 

• Use of evidence to assess health, identify priorities and develop strategies to improve the health of our communities. 
• Find assets, needs and inequities to set priorities for programs and services to improve the health of our population.   
• Demonstrate the use of data to inform a population health approach through the voices of people with lived experience and sharing stories of communities’ engagement.
